2023 1095C’s were included with your W-2 if you were due to receive one.

If you need a replacement W-2 or 1095C for this year (or any prior years) then please print and follow the instructions on the Tax Form Replacement request. Incomplete requests or requests without the appropriate fee included will not be processed. We are unable to accept electronic forms of payment for these fees, including debit or credit cards.
